Insurance Information

Please be sure to bring your insurance card with you to your initial visit.  Please bring a photocopy of the front and back sides if you do not have your card available.

A referral or prescription is not needed to utilize our services.  Anybody can see us for either 10 visits or 30 days for an injury/condition (whichever ends first) without a doctor’s referral or prescription.  After this period, a prescription is needed.  Insurance coverage for this 10 visit/30 day period varies, so be sure to contact your insurance provider to find out what exactly your therapy coverage is.  You are responsible for paying in full for your sessions at the time of service for whatever amount your insurance does not cover.

If you have a non-participating insurance, you can still be seen by a M.O.S.T. Clinician. After paying for and receiving your service, you can submit your receipts to your insurance company for reimbursement.

For those patients who are out of network or wish to pay cash, our rates are as follows:

• Initial Evaluation: $125.00
• Subsequent Visit(s): $75.00
